This form is a PDF file. Files in PDF format (filename.pdf) require Adobe Acrobat Reader to view or print them. This is a free program available for download here.
Tip: If you already have Adobe Acrobat Reader on your computer, when you press on a download link for a file in PDF format, it will open automatically displaying the file. You can then go to File, Save in order to save it to your hard drive. To print before or after saving the document, go to File, Print. If you download a PDF file without Adobe Acrobat Reader on your computer, your browser (Netscape or Internet Explorer) will automatically prompt you to save the file to your disk. Note where you save the file so as to open it after downloading Acrobat Reader. |
